Community Support Network (CSN) connects individuals in Arizona to services and resources that can help them enhance their quality of life. We partner with communities and organizations around the state to improve access to housing, education, public safety, healthy foods, and local emergency services through a variety of programs.

Eligibility and Care Services

We administer the Seriously Mental Ill (SMI) Eligibility Determination program for the state of Arizona. Behavioral health service providers can complete an evaluation for an individual and submit the SMI Assessment Packet and we’ll make an eligibility determination.
