Agency Onboarding
Thank you for your interest in becoming a partner with DWEL-AZ to access the data warehouse.
- This onboarding application is for any community-based organizations and programs, municipal agencies, and organizations requesting access to the data warehouse.

- This form is for interested and onboarding agencies requesting access to the DWEL-AZ data warehouse. If you have any questions about the onboarding process or would like to request a meeting with the DWEL-AZ Program Administrator to discuss the application process, please submit your request using the link below. If your agency is currently in the application process and needs technical assistance, please submit your request below.
Data Privacy and Security Training
This is a required training for all new DWEL-AZ users. Please refer to the New User Set-Up Requests to obtain the Building a Culture of Data Privacy and Security Attestation and the DWEL-AZ User Terms of Service Agreement.
- This training provides an overview of the DWEL-AZ data privacy and security expectations. This training helps users familiarize themselves with the privacy notice, the data security policy, and adherence to the code of ethics. This training is required for new users.
New User Set-up Requests
- A guide for user account set up and reset instructions.
- Agency Liaison form to request activation, deactivation, or make changes to a user role/type.
- Required attestation for new users. User attesting to the completion of the Building a Culture of Data Privacy and Security training video.
- Required agreement. Formal user agreement outlining data security and responsibilities as a data warehouse user.

DWEL-AZ ROI HMIS Upload Instructions
- The below documents contain the DWEL-AZ Release of Information (ROI) to disclose Protected Health Information (PHI). This form is necessary for all clients found in the data warehouse. A guide for talking to people about the data privacy and security, as well as instructions, are provided for support.
*Important* Once the ROI is signed, please upload the ROI into HMIS. See the DWEL-AZ ROI HMIS Upload Instruction forms and training video.
