Privacy Practices and Forms
DWEL-AZ is an ongoing project establishing a Statewide data warehouse to support the integration of systems of care for individuals experiencing homelessness in Arizona. The project aims to improve care coordination between homeless services, healthcare, and other systems of care. This is accomplished through gaining the individuals permission to share and link up their information between these systems, allowing staff to holistically support individuals and families seeking services.
The information we collect is important for streamlining eligibility processes, getting services to individuals who need them, improving services, and improving understanding of the needs of individuals in our community. All participating agencies with DWEL-AZ may share personal member information with each other in accordance with the user’s permission level. This may include information you have provided or information that has been obtained with your consent.
The security and privacy of your personal information is a top priority. All information in these databases is protected by passwords, encrypted technology, restricted user roles, and/or other security measures. Steps are taken to safeguard all of the information in the system.
